Advance Care Planning is for everyone\, not just those with a serious illness. An illness or injury can often come without warning. If you plan now\, you can make sure that the medical treatment you get in the future will be the treatment you would want. We will provide an overview of Illinois’ advanced care planning documents including Living Will\, Power of Attorney\, Practitioner’s Orders for Life Sustaining Treatment\, and more.
