It can be difficult to think about medical emergencies. While most people agree it is important to make plans, too many of us fail to put these plans in writing. By planning ahead, you can be empowered to ensure your wishes are heard and followed.
This workshop will encourage participants to consider their preferences for medical care in the future, in emergency situations, and for end-of-life care; and ensure they are legally documented using The Five Wishes. This document is an easy to understand, legally binding advance directive that is simple to fill out; no lawyer or notary necessary! The Five Wishes also addresses social, emotional, and spiritual needs.
Participants will receive a free copy of The Five Wishes and each wish will be explained in detail, helping attendees gather a greater understanding of the importance of documenting their preferences. Many will experience a new level of comfort considering and discussing future healthcare decisions with loved ones and healthcare professionals.
About Maureen Burns
Maureen Burns is a Community Health Worker /Promotora de Salud with JourneyCare, a nonprofit provider of hospice and palliative care. She works in and around Rogers Park, providing free community education and linking patients and loved ones to resources within their community.
