Description: This workshop will discuss advance care planning and why it is important. People are living
longer, and medical technology continues to make advancements. Having an advance directive helps to
ensure that you get the care that you want! The Five Wishes is a low-cost and legally binding advance
directive, it is easy to understand and does not require a lawyer. The document covers naming a
healthcare agent, and medical decisions and provides space for spiritual and social-emotional wishes as
well. The presentation will focus on how planning ahead can be empowering and reduce stress on our
loved ones.
Presenter Bio: Maureen Burns (she/her/ella) is a Community Health Worker /Promotora de Salud and
the Director of Community Health at The HAP Foundation. The HAP Foundation is a nonprofit leader
with a refined focus to elevate access to and increase understanding of serious illness and hospice and
palliative care through community and clinical education, workforce development, research, and
advocacy. Maureen works in Rogers Park and surrounding communities, providing free community
education, and linking patients and loved ones to resources within their community. She has over 15
years of experience working in health care and a Bachelor of Arts degree in community health, culture,
and aging.
