When we or someone we love is diagnosed with a serious illness it can be a scary and confusing time. We often avoid thinking about these situations until we have to because it can be overwhelming. We want to make sure that if you are ever needing support, you are prepared and empowered. This presentation will help explain the different kinds of support available and how to access care when needed. Learn about palliative care and how it can help at any stage of a serious illness. Learn about holistic hospice care for those facing end-of-life, even when cures fail, there is still much that can be done to support patients and their families. You don’t have to face a serious illness alone!
Participants will learn about palliative care and hospice care, and how these services can help those experiencing a serious illness. We will explain palliative care and hospice care, what they have in common, as well as how they are different. Participants will be informed on the benefits of care, eligibility, insurance coverage, and how to access care.
About our presenter: Maureen Burns (she/her/ella) is a Community Health Worker /Promotora de Salud with The HAP Foundation. The HAP Foundation is a nonprofit leader with a refined focus to elevate access to and increase understanding of serious illness and hospice and palliative care through community and clinical education, workforce development, research, and advocacy. Maureen works in and around Rogers Park, providing free community education and linking patients and loved ones to resources within their community. She has over 15 years of experience working in health care and a Bachelor of Arts degree in community health, culture, and aging. She also has experience as a caregiver.
